Diagnostic Report
* Identifying Information
Name: Francine Lewis
Date of Birth: 10/12/1945
Date of Evaluation: 11/1/2012
* Introduction
Francine Lewis, age 67, received a language evaluation on November 1, 2012 at the Northeastern State University Speech-Language-Hearing Clinic. She was referred by Dr. John Locke of Green Country Rehab Center.
* History
Francine experienced a left CVA in the middle cerebral artery territory on September 24, 2012. Prior to the CVA, she was diagnosed with hypertension, osteoporosis, and had a history of transient ischemic attacks. Francine stated that she was “depressed” over her CVA and the impact it had on her activities of daily living.
* Evaluation
Francine was administered the Boston Diagnostic Aphasia Examination (BDAE-3). The BDAE is a test for adult aphasia that assesses 5 functional areas including Conversation and Expository Speech, Auditory Comprehension, Oral Expression, Reading and Writing. In the area of auditory comprehension, Francine’s results regarding word comprehension, following commands of increasing complexity, understanding complex ideational material, and syntactic processing with the comprehension of embedded sentences were all within functional limits. However, she did have some difficulty with grammatical morphemes. In the area of oral expression, Francine’s ability to repeat phrases was moderately impaired; she presented difficulties with multisyllabic words; and she experienced frequent anomia, with attempts to self-correct. Although her syntax was within functional limits, her speech was circumlocutionary and paraphasic.
* Impressions
Due to the results obtained on the BDAE-3, the aphasia type was concluded to be conduction aphasia. Francine scored within normal limits for auditory comprehension, but her results for oral expression, excluding syntax, were all below normal limits leading to the conclusion of conduction aphasia.
